TO QUALIFY FOR THE SCHEME
Form can only be filled in with the approval of the sales consultant/manager via email with the form attached.
• This form must be filled in and returned to Preeminent Care Services before the referral is made.
• Referral must not already be on the Preeminent Care Services database.
• Referral must work 25 shifts minimum for Preeminent Care Services.
• You must be registered on our database to receive the referral.
• Form must be returned via email to the sales consultant who has authorised the scheme.
Failure to follow any of the above steps will result in the referral not being paid
